Dementia Survey

NCPC has a two year project to promote increased and better quality palliative care for people with dementia. As part of the project, we would be very grateful if you would take part in this survey to help NCPC understand better the views of people with dementia and their family carers about palliative and end of life care needs.

This survey will contribute to a key outcome for the project which is to influence national policy on palliative and end of life care services for people with dementia.  The survey is being conducted in partnership with ‘Uniting Carers’ within ‘for dementia’.

We would be very grateful if you could kindly complete the simple questionnaire below. The survey can be completed on your computer and after saving can be emailed back to us at a.khatri@ncpc.org.uk. Alternatively, please print the survey and send it back to us by post – details are provided at the end of the questionnaire.

The deadline to return the completed questionnaire is Friday 18 July 2008.
