Join NCPC's Service User Reference Forum (SURF)

What is the Service User Reference Forum (SURF)?

SURF is designed to offer people an opportunity to share their story either once or from time to time in consultations or questionnaires. This is to allow those who would like to offer their input but who are unable to or do not wish to offer a regular time commitment.

How does it work?

You can share your story anonymously by clicking here.  If you would like to contribute to future consultations about palliative and end of life care, simply tick the box and fill in your contact details and someone from NCPC will be in touch and inform you of any further work being carried out in this area.

Unfortunately we are not able to assist with individual cases of care, but if you do need assistance click here for further help.

What kind of information should I share?

At NCPC, we want to know what kind of care people need if they have a life limiting condition or are nearing end of life. We’d like to hear what you feel could be done to provide good care for people, based on your experience.

Who can contribute?

Anyone who has personal experience of end of life or palliative care.

How will this information be used?

Personal experience speaks volumes when communicating people’s palliative and end of life care needs. Two examples of how we do this are to produce case studies to illustrate the need for a particular type of service or include a quotation about a particular aspect of services.

Some examples of how we share this information are:

  • Good practice guides for health and social care professionals, to help improve standards of care.
  • To inform national guidelines about health care.
  • Government consultations about health care policies and strategies. For example the End of Life Care Strategy.

In all cases, we will ensure that the input you give us remains anonymous unless you have informed us otherwise.

When can I contribute?

Any time of day or night.
