NHS Heart Improvement Programme - Resource Kit for Cardiac Networks

This resource kit developed by the NHS Heart Improvement Programme has sections reflecting the sequence a network might follow to review and plan the provision of supportive and palliative care services for local heart failure patients.

NHS Service Improvement Online

This is an online resource to support improvement of NHS services for patients and carers. it is designed to reflect the journeys patients take through the NHS system. It is available to every NHS organisation in England.

Heart Failure and Palliative Care Services Working in Partnership: Report of a New Model of Care

The above link refers to an article in Palliative Medicine by Andrew Daley et al in which a three year collaboration between community based heart failure nurse specialists and existing palliative care services is described along with the model of care that evolved.

Better Together: Marie Curie Cancer Care and British Heart Foundation working together

Marie Curie Cancer Care and the British Heart Foundation are working together to deliver better patient care to advanced heart failure patients in Poole, Bournemouth, Airedale and Bradford. This service provides the best possible care for chronic heart failure patients, allowing them to be cared for at home by both BHF and Marie Curie Nurses.

Working Collaboratively to Meet the Palliative Care Needs of Patients with End-Stage Heart Failure

The above link refers to a poster by Dr Aruna Hodgson et al in which is based on the collaborative work being done by the specialist palliative care and heart failure services in Wigan. One of the recommendations is that the patients with heart failure and their carers should have access to professionals with palliative care skills within the heart failure team.
