NCPC survey to monitor the implementation of the End of Life Care StrategyAs part of our work to monitor the implementation of the End of Life Care Strategy in England, NCPC is undertaking a survey of SHAs, PCTs and palliative and end of life care providers. The brief survey is exploring the funding of services in light of the End of Life Care Strategy and the use of quality markers for end of life care. You can complete the survey on behalf of your organisation online here We have experienced some problems with the software used to create this survey. These have now been resolved, but if you would prefer to fill in Word versions of the forms, these are available to download here. There are separate versions for SHA end of life care leads; PCT commissioners; NHS provider organisations and for voluntary sector organisations. Please complete the one appropriate to your role and organisation and, if possible, it would be helpful if only one response per organisation was submitted. We apologise for any inconvenience this may cause.
