MDS Project Data Entry Forms 2007/086. Out-patient Clinic CareThis section is about patients who attended your out-patient (OP) clinics. An out-patient clinic, as opposed to day care, is defined by each patient having an individual appointment to see a specific member of staff (eg doctor, social worker). Do not include clinics run by another specialty, e.g. oncology, attended by specialist palliative care staff in an advisory or supporting role. Please answer for the year 1/4/2007 to 31/3/2008. All fields marked with * are mandatory. Use Tab Key or mouse to navigate through the fields. Do not use the Enter/Return key. |
