6. Out-patient Clinic Care

This section is about patients who attended your out-patient (OP) clinics.

An out-patient clinic, as opposed to day care, is defined by each patient having an individual appointment to see a specific member of staff (eg doctor, social worker). Do not include clinics run by another specialty, e.g. oncology, attended by specialist palliative care staff in an advisory or supporting role.

Please answer for the year 1/4/2007 to 31/3/2008.

6.1	All patients
	Give the total number of patients receiving out-patient care during the year, whether new or existing referrals. Count each patient only once. (This number can be obtained from the number of new patients plus those seen for the first time during the year who were first registered in a previous year)

6.2	New patients - total
	Give the number of new out-patients, i.e. those who attended an out-patient clinic for the first time ever during the year.

6.3	New patients - analysis
	Now give the number of new patients (adding up to 6.2 above) in these categories:

	a) Age on registration and sex	Female	Male	All

	b) Primary diagnosis

6.4	Out-patient attendances
	Give the total number of out-patient attendances during this year.

6.5	Out-patient clinics - total
	Give the total number of out-patient clinics held during this year.

6.6	Type of clinic - analysis
	Record the total number of palliative care clinics held during the year and the number of attendances at each type of clinic.
