MDS Project Data Entry Forms 2007/08

2. Day care

This section is about patients who attended your day centre or day care sessions. Include in your replies any in-patients who attended day care during their stay.

Please answer for the year 1/4/2007 to 31/3/2008.

2.1 All patients
Give the total number of patients registered as attending day care during the year, whether new or existing referrals. Count each patient only once.
(This number can be obtained by adding the number of new patients plus those attending for the first time during the year who were first registered in a previous year)
2.2 New patients - total
Give the number of new day care patients, i.e. those who attended day care for the first time ever during the year.
2.3 New patients - analysis
Now give the number of new patients (adding up to 2.2 above) in these categories:

Please check that each section of 2.3 totals to the figure you gave in 2.2 above

2.4 Day care sessions
Give the total number of day care sessions held in the year. The opening hours of the day centre or day care facilities on one day normally count as one session.
2.5 Day care places
Give the total number of day care places available in the year.
Calculate by adding together the number of available places at each session. Include any places reserved for in-patients, but not day care accommodation used by anyone other than patients (e.g. carers or visitors).
2.6 Day care attendances
Give the total number of day care attendances by patients in the year.
2.7 Deaths and discharges
a) Give the number of patients during the year (out of the total given in 2.1 above) who died while registered as a day care patient
b) Give the number of discharges from day care during the year, i.e. periods of day care which ended other than with the patient's death. Count as discharged if the patient no longer requires day care, or has not attended for 3 months for any other reason
2.8 Length of care episode
Give the number of completed periods of day care (adding up to 2.7a+b above) divided according to the time from first attendance to death or discharge:

2 weeks up to 1 month
1 up to 2 months
2 up to 3 months
3 up to 4 months
4 up to 5 months
5 up to 6 months
6 months to one year

i)   Give the average (mean) length of period of day care, defined as the average duration in days from first attendance to death or discharge
ii)  Give the number of patients (out of the total given in 2.1 above) who attended day care throughout the whole year, i.e. were already registered for day care on 1/4/2007 and were still registered (not discharged/died) on 31/3/2008
NOTE
Any patients who are admitted to a bed without an overnight stay on a planned basis, should be counted as day cases in the in-patient section (Section 1) of this questionnaire and not as day care attenders.
